Plecanatide – A similar IBS experimental treatment

Plecanatide & Linaclotide are two medications developed by two different firms, but both are closely treating Irritable Bowel Syndrome. Synergy Pharmaceuticals is developing Plecanatide while Ironwood Pharmaceuticals is developing Linaclotide. These drugs are aimed at those with predominant IBS constipation with abdominal pain associated with IBS.

Both the medications, Plecanatide & Linaclotide are agonists that activate the GC-C (guanylate cyclase C) receptor in the intestinal tract mucosa covering. It triggers the release of proteins CFTR (cystic fibrosis transmembrane conductance regulator) when this receptor is stimulated. Such CFTR proteins enhance intestinal material distribution by increasing water & chloride secretions while at the same time reducing liquid absorption.

These also seem to help reduce inflammation. The end result is identical to the only IBS medication currently available Lubiprostone(Amitiza), Lubiprostone operates differently, resulting in an increase in chloride & water secretions in the stomach. Linaclotide is further established in US regulatory body trials as far as effectiveness is concerned. Throughout Phase III (final phase) Linaclotide trials, in which 800 patients took part, 34% of participants receiving Linaclotide reported pain & constipation relief relative with 21% taking a placebo. At least 50% of the pain was decreasing relative to 37% for placebo. Six percent of the respondents reported diarrhea that was too serious to exit the experiment fully. Plecanatide is planning for clinical tests in Phase II. It hasn't done a major test on its efficacy yet.

Essentially, these medications seem to be more sophisticated types of laxatives with the added benefit of potential pain relief and a reduced chance of building up resistance over time, resulting in the ineffectiveness of the more common stimulant-based laxatives. Their most possible application case will boost the effects of constipation when traditional laxatives and/or dietary changes fail.
